This tool can be used to help nephrologists and dialysis staff identify and assess candidates for home dialysis therapies (PD and HHD). It is designed to sensitize clinicians to key issues about who can use home dialysis.

What’s happening inside of me?
Living with kidney disease is often overwhelming and involves a lot of information for patients and families. To help children, youth and families learn how to live well with chronic kidney disease, the Children’s Hospital, Health Sciences Centre, Winnipeg, Canada developed this Kidney Health Education Resource (KHER).
There are 15 modules covering many topics about kidney health. Each module has a pre-school section, a school-age section and an adolescent section. There is a separate booklet for families or caregiver and a fun activity sheet with each module.There 15 modules covering many topics about kidney health. Module 16 is a glossary of all highlighted terms in the modules with easy to understand definitions. The pictures in the glossary provide clues or reminders to help connect with the meaning of the words we use often.
Each of the 15 modules has a corresponding Facilitator Resource for healthcare providers outlining what you should know in order to use these modules/tools effectively.
The facilitator resource provides background information about facilitated learning and motivational interviewing; key messages and clinical targets specific to the module; and learning objectives for each developmental level. There are suggestions for helping the learner move through the materials and it is designed as a working tool with space for inserting your own ideas and notes.
